RSI happens because your wrists spend hours bent in positions they were never designed for. The problem isn't the keyboard. It's the angle your wrists maintain while typing.
When you type, two things go wrong. Wrist extension (bending upward) compresses the carpal tunnel — the passage in your wrist where nerves and tendons live. Bend your wrist upward, and pressure inside the carpal tunnel increases. Blood flow to tendons drops. Inflammation starts. Do this for eight hours a day, and those tendons begin to fray.
Ulnar deviation (bending your wrist outward) constricts the carpal tunnel further. Standard keyboards force both positions at once. Your fingers float upward to reach keys while your hands angle outward on the keyboard. This isn't a sudden injury. It's damage that builds week by week.

Neutral Wrist Position: The Exact Setup
Neutral wrist position means your hands and wrists are straight, in line with your forearms. Not bent up. Not bent down. Not angled to either side.
Three things need to be in alignment: desk height, chair height, and keyboard placement.
Step 1: Chair Height
- Sit all the way back in your chair, back against the backrest
- Feet flat on the floor
- Knees at roughly 90 degrees
- If your feet don't touch the floor, use a footrest — dangling legs create shoulder tension that travels to your wrists
Step 2: Desk Height (The Critical Measurement)
Your elbows should be at 90 degrees when your arms hang naturally at your sides. This single measurement matters most.
- Sit in your chair with your feet flat
- Let your arms hang naturally at your sides
- Bend your elbows to 90 degrees
- Your desk surface should meet your forearms when you reach forward — this is your correct desk height
If your desk is too high, your elbows angle upward, and your wrists bend into extension. Too low, and you hunch forward, creating shoulder tension that radiates to your wrists.
Step 3: Keyboard Position
- Center your keyboard on the letter "B" — aligned with your belly button, not the number pad
- Keep it flat, or tilted so the front edge is slightly higher than the back (called negative tilt)
- Close enough that your elbows stay near your sides, not reaching forward
- Forearms should be parallel to the floor or angled slightly downward when typing
Don't raise the back of your keyboard. Raised rear feet increase wrist extension — the exact mechanism that causes RSI. If you've been doing this, fold those feet down today.
Step 4: Monitor Height
Wrong monitor height forces your wrists to compensate. Lean forward to see a low monitor and your wrists extend. Crane your neck to look up, and tension radiates down both arms.
- Top of your monitor at or slightly below eye level
- Arm's length away — roughly 20 to 26 inches
- If you wear bifocals or progressive lenses, lower it one to two inches to prevent tilting your head back
Step 5: Mouse Position
Your mouse should sit at the same height as your keyboard, directly beside it.
- Keep your hand in a straight line with your forearm — no bending at the wrist
- Avoid gripping with your palm flat down; a neutral handshake position reduces strain
- Vertical mice or trackball mice cut strain significantly compared to traditional flat mice
Micro-Breaks: What Actually Works
Good positioning prevents injury. But positioning alone isn't enough if you type eight to twelve hours daily without breaks.
Your tendons need recovery time. No equipment overrides continuous overuse.
Use the 20-20-20 rule: every 20 minutes, look away from your screen for 20 seconds and focus on something 20 feet away. This also forces you to stop typing.
During those breaks:
- Stand and move
- Roll your shoulders backward three times
- Shake your hands loose — floppy, not tight
- Stretch your wrists: gently pull your fingers back toward your body, hold 15 seconds in each hand
Every two hours, take a five-minute real break. Walk. Get water. Break the pattern of repetition. Micro-breaks allow tendons to recover before inflammation starts. You're preventing damage, not treating pain.
The Truth About Ergonomic Keyboards
You don't need a $300 keyboard to prevent RSI.
Ergonomic keyboards can reduce strain by 10 to 15% — but only if your setup is already correct. They will not fix a fundamentally wrong position.
What matters most, in order:
- Positioning your current keyboard correctly ($0)
- Taking breaks ($0)
- Adjusting your chair and desk height (maybe a $20 to $60 footrest)
- Reducing daily typing volume
- Ergonomic equipment ($100 to $500+, only helps if steps 1 through 4 are already done)
Most people skip steps one through four and buy an ergonomic keyboard. They still have wrist pain because the setup itself is wrong.
Your Workspace Is a Positioning System
Cable clutter behind your desk creates reaching and twisting. Both force your wrists to compensate. Organized cables — routed and out of reach — means a stable wrist position throughout the day.
Cable management isn't an aesthetic choice. It's a positioning layer.
When your workspace is built intentionally, your body rests in neutral.
